Provides direct, individualized support to participants with developmental disabilities to promote greater independence, productivity, family involvement, and community inclusion, as outlined in their service plan. Services are delivered one-on-one with the participant, either in the home or within community settings, and are designed to support meaningful participation in everyday activities.
Community-Based Supports assist individuals in learning and applying essential life skills in real-world environments.
Examples of Community-Based Supports may include but are not limited to:
- Support to attend community events, classes, or recreational activities
- Assistance with daily living tasks in community settings (e.g., eating, hygiene, or dressing as needed)
- Help with ordering food from menus and making appropriate selections
- Support with purchasing items and handling basic money transactions
- Training in basic cooking and meal preparation skills
- Instruction in laundry skills and household task completion
- Travel training and learning how to safely use public transportation
- Support in developing appropriate social skills and community etiquette
- Assistance with accessing and participating in community resources and activities
- One-on-one tutoring or skill-building support
- On-the-job or community site support with basic self-care, social skills, and daily living activities
