Provides direct, person-centered assistance to individuals with developmental disabilities to support them in achieving and maintaining greater independence, productivity, family involvement, and community inclusion as outlined in their individualized service plan. Services are delivered in the participant’s home or community setting and are designed to promote skill development, independence, and meaningful participation in daily life.

Individual Supports may include but are not limited to:

  • Assistance with community-based activities and participation in community life
  • Training, prompting, and supervision as individuals learn daily living skills
  • Support with basic self-care tasks such as hygiene, grooming, and dressing
  • Assistance in developing and improving social skills and interpersonal interactions
  • Support with activities of daily living (ADLs) to promote independence at home and in the community
  • Guidance in following routines, schedules, and household responsibilities
  • Support in learning and practicing safety awareness skills in home and community settings
  • Assistance with communication and decision-making in everyday situations
  • Encouragement and coaching to build independence and confidence in daily tasks

The service is individualized and focused on helping each person build the skills needed to live as independently as possible while staying engaged in their home, family, and community life.
